Requirements for Foreigners of Pakistan Origin

5 year multiple entry visa is issued to US citizens of Pakistan origin only with proof of Pakistan origin.

• Visa Form duly completed and signed by the applicant/ward
• Valid Passport along with a copy (passport must be valid for at least 6 months)
• Proof of Pakistan origin (copy of passport CNIC/NICOP/CRC/Birth Certificate/’Bay’ Form/Birth Registration Certificate (Form-S)).
• For children, attach copy of Birth Certificate and Father/Mother's Pakistani passport or CNIC/NICOP.
• If an applicant is married to a person of Pakistan origin, attach a copy of Marriage Certificate and Spouse’s Pakistani passport or CNIC/NICOP.
• 2 recent passport size photographs with white background (front view) not older than 3 months
• An applicant holding passport of a country other than US must provide proof of legal resident status in the US (copy of Work Authorization, Alien Registration Card etc.)

(Fee is non refundable. Fee should be in the form of money order or cashier’s check payable to “CONSULATE GENERAL OF PAKISTAN”. NO CASH OR PERSONAL CHECKS ARE ACCEPTED.)

If applying through mail, a self addressed prepaid envelope is mandatory. (USPS Express or Priority mail envelopes with delivery confirmation only)

NOTE: Granting of Visa is the sole prerogative of the Mission.
