Guidelines for applying for NICOP

Duly  filled NICOP application
1. Applicant signature on the application
2. Affix thumb impression at the specified place on the form
3. Two passport size photographs (white background facing towards the camera)
4. Present address in United States of America
5. Permanent address in Pakistan
6. Date of Birth must be in Pakistani format (DD/MM/YYYY)
7. Phone number in the United States
8. Attestation of form by any CNIC/NICOP holder

Note: Applicants are advised to provide a landline or mobile contact number so that NADRA staff can contact you in case of any problem with their application. Unavailability of a valid contact number may cause indefinite delay in processing of NICOP application.

New NICOP (First Time)

Required Documents:

1. Duly filled NICOP application form
2. VALID Pakistani or US/Foreign Passport photocopy (First 4 Pages) (No Exceptions)
3.  Computerized ID Card (CNIC) photocopy or
4.  National ID Card (Old ID Card) photocopy or
5.  Form “B” photocopy or
6.  Pakistani Birth Certificate photocopy or
7.  Matric/School Leaving Certificate photocopy or
8.  Citizenship Certificate from Ministry of Interior (MOI)
9.  Money Order for US$ 35.00 payable to “Consulate General of Pakistan New York”. Cash / personal checks are not acceptable.
10.  Two Passport size pictures with white background only (facing towards camera)
11.  A stamped, self addressed return envelope for delivery of receipt / acknowledgement of your NICOP application when applying through post
12.  At least one NADRA Urdu Card or NICOP card copy is required as reference from first blood relative (father, mother, brother, sister, son, daughter, husband, wife, or Paternal Grand father/ mother) if applying first time on the basis of old ID card.

For Children Born in United States/Foreign Country:

1. Duly filled NICOP application form
2. VALID passport photocopy
3. Birth certificate photocopy
4. Parents’ NICOP/CNIC/NIC card photocopies
5. Money Order for US$ 35.00 payable to “Consulate General of Pakistan New York”
6. Two Passport size pictures with white background only (facing towards camera)
7. A stamped, self addressed return envelope for delivery of receipt / acknowledgement of your NICOP application when applying through post.
