Manual Passport

  • Complete Form-A

  • Notarized copy of CNIC/NICOP
  • Applicant less than 18 years of age should provide notarized copy of Registration Form-B or CNIC/NICOP
  • Original expired passport and one copy
  • 4 photographs
  • Renewal fee
  • Notarized copy of Nikah Namah in case applicant wants to have husband's name on passport.

Note: Passport with one year validity will be issued in the absence of CNIC/NICOP. The name appearing on the new passport would strictly be as per old passport or national ID card, except in the case of marriages on the submission of notarized copy of Nikah Namah.

In case the applicant does not have original CNIC/NICOP, then HE/SHE should submit, original old national ID card ( with two copies) along with any 1 documents out of following 7 mentioned below :-

  • Original Birth Certificate (with two copies)
  • Original Citizenship Certificate (with two copies).
  • Original Form-B (with two copies).
  • Original Domicile (with two copies).
  • Original Pakistani Educational Certificates minimum 2) (with two copies).
  • Original Expired and previously issued Passport (with two copies).
  • In case applicant has traveled on mother's passport, then mother's original passport on which the applicant was endorsed. (with two copies).
  • Note:
  • in case of non-fulfillment of requirements mentioned above, the passport will not be issued. Such cases would then be referred to concerned authorities in Pakistan for verification and clearance, before issuing the passport, which will take minimum 10-12 weeks.
  • A six (6) months restricted validity passport will be issued in lieu of lost one, which will be extended for full 5 years validity on the receipt of verification and clearance from concerned authorities in Pakistan.

  • Form-A
  • Notarized copy of Mother's Pakistani passport on which applicant was endorsed or Notarized copy of birth registration certificate (Form-S)
  • Notarized copy of child's CNIC/NICOP OR Form ? B (if born in Pakistan)
  • Notarized copy of Father's valid Pakistani Passport and CNIC/NICOP
  • No Objection Certificate from mother with copy of her passport
  • 04 photographs

IF the Mother's Passport is valid

  • Complete Form-C
  • Mother's valid original Pakistani Passport with copy
  • Notarized copy of child's birth registration certificate (Form-S)

  • Child's 2 photographs
  • Endorsement fee

IF the Mother's Passport is expired

  • Form-A completely filled
  • Mother's expired Pakistani Passport with copy
  • Mother's Passport renewal fee
  • Mother's 4 photographs
  • Notarized copy of Mother's CNIC/NICOP
  • Notarized copy of child's birth registration certificate (Form-S)
  • Child's 2 photographs

  • Complete Form-C and 2 photographs
  • Original passport with copy
  • Notarized copy of CNIC/NICOP
  • Endorsement/Amendment fee

Important Note: - No fee id charged to extend the limited validity passport on the submission of notarized copy of CNIC/NICOP
